AIR-CLINIC ROCKS! It’s good to be back with another interesting episode of Online Ward rounds, a special programme for medics on the blockchain. It is a part of @Air-Clinic's sub-project,
SMSI (steem medics support initiative).

A 17-year-old girl with hypertension of 160/100 is found to have a femoral pulse delay.

  • What is your diagnosis? (And what is the basis for making your diagnosis?)

Case 2

A 40-year-old woman presents with malaise and low-grade fever for 2 weeks. She recently had treatment for a dental abscess. On examination, she is noted to have splinter hemorrhage and an early diastolic murmur.

  • What is your diagnosis? And what is the basis for making the diagnosis?
  • What are your DDx?

  • How would you work up the patient?

  • What are your management modalities for this case?

Tell us about yourself

My name is Ogbonna Henry Nnaemeka .I am Biochemistry graduate from Michael Okpara University of Agriculture, Umudike ( MOUAU). My steemit username is @donnest. I am a lover of the beautiful game of football, I take my spare time to enjoy the game and read some good books too.

Why you love @air-clinic

I love air-clinic because of the opportunity it gives and helps medical and health science students, to learn more about their profession and also become better Steemians.

High and low point on steemit

Firstly, my low points in steemit were the times I had constant low payouts and no interaction on my blog. When I discovered @steemjet and @dimimp , It marked a new beginning to my steemit journey. Also,I am grateful to @steemnaira who lifted me in the turbulent times.

How do you balance your steemit life and real life

That was a difficult one at first, combining work and coming online to catch up with stuff on steemit was a bit challenging. But as time went on, I started becoming used to the activities. I just use my time well whenever it comes to the both. Giving steemit its time and work its time, that's the magic.

Motivation to study your career.

Though, I rooted for something else at onset. But Basically, what motivated me was my O'level achievements, I was very good in biology and chemistry. I felt they were my power zone and I had to explore more, though I later found out that Biochemistry was more than just that. But, the knowledge of the two helped a great deal.

Challenges/survival story in your specialty.

Biochemistry as a course was difficult, especially having to get used to pathways, enzymes and the rest, but it became interesting when I started developing interest. That's to say interest was very key to my survival and success in my discipline

Final words

The medical and health sector is one sensitive area that requires commitment, dedication, and honesty. Our job predominantly deals with lives, I encourage each and every one of us to hold this at heart and ensure we offer our best services, knowledge, and skills to save lives.

For case 1
My diagnosis is aortic coarctation with differential of oartic compression, narrowing or stenosis from tumor, atherosclerosis, etc.

Reason is hypertension in an adolescent plus femoral delay.

Investigation is basically radiological... X-rays, MRI,

Treatment is conservative and symptomatic with inotropics and diuretics depending on symptoms /complications. Surgery is the last resort.

For case 2
My diagnosis is Infective endocarditis, with differential of rheumatic fever /heart disease, valvular heart disease.

Investigations includes CBC, blood culture, CXR, echocardiography,

Treatment is with antibiotics and treatment of complications.
