#AIR-CLINIC ROCKS! It’s good to be back with another interesting episode of Online Ward rounds, a special programme for medics on the blockchain. It is a part of @Air-Clinic's sub-project,
SMSI (steem medics support initiative).

FOR THIS WEEK, HERE ARE THE CASES FOR YOU TO ATTEMPT.

Case 1

A 57-year-old teacher says she has facial pain, especially in the temples at night. On the right side of her face, it throbs. For the past three weeks, she felt unwell and had to miss classes. Combing her hair has become painful.

  • What is your diagnosis? (And what is the basis for making your diagnosis?)
  • What are your DDx?
  • How would you work up this patient?
  • How would you manage this patient?

Case 2
A 43-year-old mechanic complains of left-sided facial pain. The pain is stabbing and runs up and down his face, especially at meal times. He has been to the dentist, but the dentist has found his teeth to be in good order

  • What is your diagnosis? And what is the basis for making the diagnosis?
  • What are your DDx
  • How would you work up the patient?
  • What are your management modalities for this case?

Case 3
A 48-year-old man has a normal memory on examination, wife complaints of him becoming aggressive lately and talking about their sexual life publically.

I am Ajasa Bolaji
A medical student of Ladoke Akintola University of Technology, though still in my preclinical days
A steemian by the username @bcrafts
A poet by nature, reader and an apt writer.
Once in a while a teacher as I help newbies on the path of steemit.

Why you love @air-clinic

I love air-clinic because the platform encourages both the medical and paramedical in Nigeria on the Steemit community, giving them the necessary assistant they need to be a good steemian.

High and low point on steemit

Highs of steemit would be when Steem and SBD were mooning, reaching to unprecedented heights
Lows would be when I joined the platform; I really suffered as I had no help until I met community projects like @steemnaira, @airhawk, @air-clinic and so on

How do you balance your steemit life and real life

This is a tough question, it ain't easy fam. As a medical student in preclinical, I am pretty sure we all know that it ain't easy. Reading Keith Moore, Guyton and co no easy. It pretty tough but I try as much as possible to balance both by writing on what I study.

Motivation to study your career.

One thing that has always spurred me on this path is the ability to serve humanity, helping individuals who need help health-wise and maybe for the money also.

Final words

If you are planning on being a medic, all that they told you Issa lie. It’s twice as tough as they said. All you need is to work extremely hard. It’s not easy being in this kind of study, takes your time, social life, and drains your energy.

Case I
everything seems to indicate that it is a trigeminal neuralgia, due to the characteristic of pain and hyperesthesia.
it can be a migraine, also a headache due to Arnold's neuralgia.

Medical management includes oral corticosteroids and B-complex vitamins, including gabapentin.

Case 2

the findings also seem to correspond to trigeminal neuralgia, probably due to involvement of the lower alveolar branch of the tooth

Case 1 is not trigeminal neuralgia.,but it can be a differential., the corticosteriod line of management is in order..

I think case 1 is temporal arteritis.

In fact it can be a temporal arteritis, due to age and the characteristics of the pain. It would be necessary to do a tomography or resonance and arteriography to corroborate or discard, and in any case while the diagnosis is obtained, I maintain the proposed treatment. Greetings to you.
