There are not many times or few times when as a doctor you have the task of caring for friends and family, I can rarely say that it has happened to me and I have to feel lucky because as doctors we always imagine the worst prognosis. It's a double-edged sword sometimes to know so much in your area. One of those times that I remember so fondly is the surgery performed on a great friend: Héctor.
While the circumstances I was in were not the best if it is true that they healed with time and surgery. A surgery I had the honor of being in charge of.

No muchas son las veces ni pocas en las que como médico te toca la tarea de atender a amigos y familiares, rara vez puedo decir que me ha pasado y he de sentirme afortunado ya que como médicos siempre imaginamos el peor pronóstico. Es un arma de doble filo a veces saber tantas cosas en tu área. Una de aquellas veces que con tanto agrado recuerdo es la cirugía realizada a un gran amigo: Héctor.
Si bien las circunstancias en las que estaba no eran las mejores si es cierto que sanaron con el tiempo y una cirugía. Una cirugía de la cual me tocó el honor de estar a cargo.

Case of Surgeon:
An accident...
At the beginning of 2017, after a heavy fall on a motorcycle, Hector, 42 years old, suffered a severe traumatism to his wrist. Trying to maneuver in the fall, he tried to place his hand so as not to hit his body and the impact caused him great pain. At that time and after the first aid, where it was evaluated that he did not have another major injury, he went to the hospital emergency room.
It was a few hours while they were treating him, because that week an epidemic of Dengue fever, a very autochthonous disease of our country, was haunting many people, so some emergencies were caused by treating these patients with high temperatures due to fever.
Finally and after a painful wait until an analgesic (a pain reliever) is placed, an x-ray of the left wrist is taken where the orthopedic surgeon immediately encounters a slightly complex but common wrist fracture, specifically to that area of the lesion we call a distal third radius fracture.
Although sometimes you suspect something bad and are almost certain that it is, you always hope to be told otherwise. And that afternoon Hector knew that he most likely had a fracture but he didn't believe it until they told him.
Caso de Cirujano:
Un accidente...
A inicios del año 2017 luego de una caída aparatosa en motocicleta Hector, de 42 años de edad sufre un traumatismo de fuerte intensidad en la muñeca. Pues tratando de maniobrar en la caída intentó colocar su mano para no pegar con su cuerpo y el impacto le ocasionó un gran dolor. En ese momento y luego de los primeros auxilios donde se evaluó que no tuviese otra lesión de importancia acudió a la emergencia del hospital.
Fueron algunas horas mientras lo atendieron, pues esa semana una epidemia de Dengue, una enfermedad muy autóctona de nuestro país estaba acechando a muchas personas, por lo que algunas emergencias se derivaban por atender a estos pacientes de altas temperaturas por la fiebre.
Finalmente y luego de una dolorosa espera hasta que le colocaron un analgésico (un calmante) se realiza una radiografía de la muñeca izquierda donde el cirujano ortopedista de inmediato se encuentra con una fractura un poco compleja pero común de muñeca, específicamente a esa zona de la lesión nosotros le decimos una fractura de tercio distal de radio.
Aunque a veces sospechas algo malo y estas casi seguro de que es así, siempre tienes la esperanza de que te digan lo contrario. Y esa tarde Héctor sabia que lo más probable era que tuviese una fractura pero no lo creyó hasta que se lo dijeron.
Long wait
Five months after the fracture, Héctor still did not have surgery, because the resources for such a type of surgery (which are very common anywhere in the world) are very expensive in Venezuela for the average worker. But it's already a topic I've talked about in previous publications and I don't want to reiterate.
It was after the joint effort of his family and great friends that he managed to get the budget for the purchase of the material necessary for the surgery: we call it "flying radio plate" a term used for the placement of a metal T-shaped material on the anterior face of the distal end of the wrist bone.
The human body has a great capacity to remodel and repair any kind of damage of its own, and Hector's case was no exception, in fact, after taking an X-ray after so long (since these cases are operated on in the first days) we could notice that his fractured wrist had been naturally sealed... But...... The problem is that in an "inappropriate" position, and this is due to the type of fracture. Something we normally expect after all this time.
Larga espera
Cinco meses después de la fractura, Héctor seguía sin operarse, pues los recursos para un tipo de cirugía como esta (que son muy comunes en cualquier parte del mundo) son muy caros en Venezuela para el trabajador común. Pero ya es un tema que he hablado en publicaciones anteriores y no quisiera reiterar.
Fue luego del esfuerzo en conjunto de su familia y grandes amistades que logró conseguir el presupuesto para la compra del material necesario para la cirugía: nosotros le llamamos “placa volar para radio” un término usado para la colocación de un material metálico en forma de T en la cara anterior del extremo distal del hueso de la muñeca.
El cuerpo humano tiene una gran capacidad para remodelar y reparar cualquier tipo de daño propio, y el caso de Héctor no fue la excepción, de hecho, tras realizarle una radiografía luego de tanto tiempo (ya que estos casos se operan en los primeros días) pudimos notar que su muñeca fracturada se había sellado naturalmente… Pero…. El problema es que en una posición “no adecuada”, y esto es debido al tipo de fractura. Algo que normalmente nos esperamos luego de tanto tiempo.

The surgery
A vicious healing of a fracture of the distal third of the radius bone, a complication that would result in the need to refracture the bone and align it in a correct position for the placement of the metal plate. Bone grafting is also necessary for these procedures, as much bone tissue is naturally lost when the fracture is not treated in time. In Hector's case, some of the pelvic area, a very common site for the removal of this type of graft, was extracted with very good results.
The surgery took about one and half hours, in short, to open the skin and soft tissues, to reach the focus of the fracture, to delimit the area, to fracture again with great care to injure the main arteries of the wrist, to place the plate with its screws in the correct place and with the greatest precision, to place the bone graft, to close and clean it.
After the surgery we were seeing the results on the X-ray and we really found them very satisfactory. The next day I introduced myself to Hector as his surgeon and guide for the next few days of treatment, rehabilitation and care, making a great friendship.
La cirugía
Una consolidación viciosa de una fractura del tercio distal del hueso radio, una complicación que traería como consecuencia la necesidad de volver a fracturar el hueso y alinearlo en una posición correcta para la colocación de la placa metálica. Para estos procedimientos hace falta también la colocación de injerto óseo, ya que se pierde de forma natural mucho tejido óseo cuando la fractura no es tratada a tiempo. En el caso de Héctor, se extrajo un poco de la zona de la pelvis, un sitio muy común para la extracción de este tipo de injertos, y con resultados muy buenos.
La cirugía constó de aproximadamente 1 hora y media, en resumen abrir la piel y los tejidos blandos, llegar al foco de la fractura, delimitar la zona, volver a fracturar con gran cuidado de lesionar las principales arterias de la muñeca, colocar la placa con sus tornillos en el sitio correcto y con la mayor precisión, colocación de injerto óseo, cierre y limpieza.
Luego de la cirugía estuvimos viendo los resultados en la radiografía y realmente nos parecieron muy satisfactorios. Al día siguiente me presente ante Héctor como su cirujano y guía en estos próximos días de tratamiento, rehabilitación y cuidado entablando una grandiosa amistad.

Patient or friend?
Héctor is a high-performance athlete, he lifts weights and before the accident he went to the gym very frequently. It was a challenge to be able to control that he didn't make exaggerated movements or exert himself over his limit after a delicate surgery. That's why he always kept in touch with me for any suggestions first.
As time went on, the improvement was evident, although sometimes it was rebellious and the bandage was removed before time. His healing was noticeable, so much so that he kept thanking me for it, he even gave me tickets to go to the cinema for free with my beautiful girlfriend @gatoconbotas, which we enjoyed.
When you do your work with love it's no longer work.... it's an uncontrollable passion. And helping people like Hector made us great friends.
Unfortunately, like many Venezuelans, he had to leave the country in search of better living conditions. He currently lives in Spain and has shared with us a video of his great recovery.
¿Paciente o amigo?
Héctor es atleta de alto rendimiento, levanta pesas y antes del accidente acudía de manera muy frecuente al gimnasio, fue todo un reto poder controlar que no hiciera movimientos exagerados ni hiciera fuerzas por encima de su límite tras una delicada cirugía. Es por eso que siempre se mantuvo en contacto conmigo para cualquier sugerencia antes de nada.
A medida que avanzaba el tiempo era evidente la mejoría, si bien a veces era rebelde y se retiraba el vendaje antes de tiempo su cicatrización fue notoria, tanto que no paraba de agradecérmelo, incluso llegó a darme unas entradas para acudir gratis al cine con mi hermosa novia @gatoconbotas, las cuales disfrutamos.
Cuando haces tu trabajo con amor ya no es trabajo… es una pasión incontrolable. Y ayudar a gente como Héctor nos hizo grandes amigos.
Lamentablemente al igual que muchos venezolanos tuvo que partir del país en busca de mejores condiciones de vida. Actualmente vive en España y nos ha compartido un vídeo de su gran recuperación.

Hii sir @drmaizo, I am @vinaykumarr, a medico from India. Your post is really informative and I had some doubts regarding the surgery, two years back my friend met with an accident and he had same kind Of fracture in the distal part of radius and even a carpel bone.
I don’t know why bones grafts are taken only from the iliac bone, can you please tell me.

Hi @vinaykumarr, a pleasure to greet you friend colleague, I am glad you liked my post and I will gladly answer your question. Actually, we can take bone grafts from various places on the body.
Most of the time we use "iliac crest" because it is a relatively easy and safe technique that offers a greater amount of bone graft than other sites, which is important in a fracture reconstruction surgery.

Cuando roté por traumatología tuve la oportunidad de entrar a varias cirugías como esta, en la que la consolidación viciosa ya había tomado parte y le daba más trabajo al equipo y siempre salía fascinada. Que maravillosa la forma en la que el médico manipula el daño para causar el beneficio del paciente. Excelente trabajo ¡Saludos!
Gracias @ths, es muy común verlas en nuestro entorno. Y probablemente por la falta de insumos se haga cada vez más frecuentes, incluso muchas veces se valora el beneficio entre una cirugía de este tipo o mantenerlo con la complicación pero limitado, más que todo en personas de tercera edad que tienen comorbilidades.
