The way I have been changing my Steem and Steem Dollars to fiat is transferring to bittrex.com and selling it for bitcoin, then transfer the bitcoin to a BitPay debit card which is USD based.
I have also used Blocktrades.us which is faster but costs a bit more in fees.
If you live in another country there are other debit cards to choose from that are Euro or British Pounds based like Cryptopay, SpectroCoin, Xapo, Shift, and Bitwala.
