Are tier 2 and 3 transitions automatic, or do we have to notify someone once we've made 4+/7+ votes a week?

We count the upvotes each week, normally on Monday/Tuesday depending on your timezone. So that is when we update all the upvote tiers.
We normally recommend setting up steemvoter or steemdunk. That way you will never miss a vote and always stay in T2. =)
