왜 한국에서는 모르핀의 사용을 꺼려하는 것일까? Why Korean People Deny Morphine?

안녕하세요, 라멘걸입니다. 최근 주말에 갑자기 병원에 가야 할 상황이 생겨 대학병원 응급실을 방문했는데요. 응급실은 몇번인가 방문한적이 있지만 이번처럼 바쁘게 돌아가는 응급실의 모습은 처음 본거 같아요. 담당 의사선생님을 2시간정도 기다려야 했기에 치료시간까지 합쳐서 응급실에서 3시간 정도를 머물렀습니다. 정말 다양한 환자들을 보았고 그 안에서 한숨돌릴틈도 없는 의사, 간호사님들을 보며 많은 생각이들었습니다.

응급실이 워낙 작았기에 모든 상황을 지켜봐야 하는 수 밖에 없었는데, 한 중년의 아저씨께서 응급차에 실려오셨고, 이야기를 들어보니 8.5톤트럭과 사고가 있었다고 합니다. 고통스럽게 울부짖는 아저씨를 2시간동안 지켜보는게 너무 힘들었어요... 이런 장면은 티비에서는 많이 본 적이 있지만 제 눈앞에서 벌어지니 고통이란게 얼마나 끔찍한 것인지 뼈져리게 느껴지더라구요. 그렇게 고통스러워 보이는 환자분을 보며 왜 의사는 강한 진통제를 놔주지 않는걸까? 생각이 들기도 했습니다. 물론 의학적인 이유는 제가 알 길이 없지만, 우리나라는 모르핀같은 강한진통제를 잘 사용하지 않는다는 이야기를 얼핏들은적이 있었기에 집에 돌아와 자세히 찾아보게 되었습니다. 결과는 놀라웠습니다.

우리나라는 이름이 주는 거부감 때문에 마약성 진통제의 사용률이 높지 않다고 합니다. 세계보건기구(WHO)에 따르면 국내 사용량은 모르핀으로 환산했을때 환자 1인당 연간 연간 45㎎에 불과하다고 합니다. 미국(693.44㎎), 영국(334.52㎎)은 물론 세계평균(58.11㎎)보다도 낮은 사용률인것이죠. 통증을 효과적으로 다스릴 수 있는 방법이 있는데도 안쓴다는 것입니다.

특히 암환자의 통증은 당사자가 아니라면 상상도 할 수 없을 정도로 심하다고 합니다. 따라서 암환자의 통증에는 마약성 진통제를 써야 하는데 일반적으로 마약성 진통제는 가망이 없어 고통이라도 덜어주려는 약물이며 중독성이 강하고, 나중에 더 큰 통증을 못잡는다는 선입견을 가지고 있는 분들이 많다고 하는데 이는 오해라고 합니다. 오히려 비마약성 진통제로는 통증 개선에 한계가 있고, 그 중에서도 소염진통제는 오래 쓰면 위장이나 신장이 손상되는 반면, 마약성 진통제는 그런 손상이 없다고 합니다. 또한 마약성 진통제는 통증 조절효과가 뛰어나며, 쾌락 자극보다 통증 감소 효과가 커 중독될 확률도 극히 낮다고 하네요. 그런데 우리나라는 마약성 진통제에 대한 막연한 두려움 때문에 세계평균의 절반도 쓰지 않을 정도로 너무 아끼는 것이죠.

‘마약’은 마취작용이 있는 약이라는 한자에서 유래되었는데요, 악마의 약 마약으로 오인되고 있는거 같아 안타깝습니다.

Hi chingus! this ramen girl. I recently visited a university hospital’s emergency room because of a situation that I had to go to the hospital suddenly on the weekend. I've visited the emergency room several times before, but I've never seen an emergency room like this busy. I had to wait about 2 hours for my doctor coming, so I stayed in the emergency room for about 3 hours including the treatment time. I saw a lot of different kind of patients and also I saw doctors and nurses who seems no time to even breathe in.

The emergency room was so small that I had to watch every situation. One patient who middle - aged man came to the emergency room. It was so hard to watch the painful man for three hours... Sounds like he’s got an accident with 8.5ton truck. I've seen a lot of these scenes on TV, but it was the first time watching in real. This patient kept yelling and mourned loudly because of the pain. So I thought why don’t the doctor give him a strong pain killer? Of course, I don’t know about medical reasons, but I’ve heard of that Korea doesn’t use a lot of strong pain killer such as morphine. So I started researching for that reason.

In Korea, people repulsed by use narcotic pain killer. The reason why the name of narcotics reminds them a drug. According to the World Health Organization (WHO), Korea use only 45mg per year per patient when converted to morphine. It is lower than the world average(58.11mg), the United States (693.44mg), the United Kingdom (334.52mg). They don’t use narcotic analgesics enough even if they have the way to manage the pain effectively.

In particular, the pain in cancer is unimaginably brutal so people saying that you can’t even imagine the pain if you’re not a cancer patient. Therefore, for cancer patients should use narcotic pain killer. In general, Korean people have a prejudice of morphine that It’s addictive, make your cancer worse or can not get much effect later. This is a complete misconstruction.
Rather, non-narcotic pain killer have limitations in the improvement of pain and NSAIDs(anti-inflammatory pain killer) which is one of the most famous non-narcotic pain killer in Korea, could damaged your stomach and kidneys if you use it for long-term, while narcotic pain killer don’t have such damage. In addition, narcotic pain killer are excellent in pain control, and they are less addicted than pleasure stimuli. However, because of the fear of narcotic pain killer, Korea don’t use morphine as much as half the world average which is sad.

In Korean, the word for 'Drug' from the kanji means medicine with narcotic effect. But it seems like people are mistaken by the drug of the devil which is same pronunciation as narcotic effect.
