I understand what you say about family shame, years ago I had an alcoholic uncle who bothered the days of all his brothers, asked for money, spent it on alcohol, exposed himself to walking drunken streets and the relatives feared that a vehicle would run over him. Additionally, his brothers watched with pain as he wasted his days, years passed and they could be calm when my uncle died (of natural causes). So I can understand what you are exposing about your relative, thank you for participating in our dynamic and telling your family history.
