Characteristics and classification of moulting injuries

Moulting injury is a traumatic injury in which a soft part of a tissue is severely damaged to reveal bone tissue. Read on to learn more about this damage!

In the medical field, moulting injury is a traumatic injury in which the soft tissue of the skin is torn over a large area to expose the bone, and it mainly affects the limbs. Most do not have a solution to the consequences of this damage. Traumatic injuries to the head or torso are usually fatal. Let's learn more about the characteristics and classification of moulting injuries.

Classification of moulting injuries

There are various classifications depending on the severity and extent of the injury.

Classification of soft tissue damage can help to understand the actual extent and consequences. Healthcare professionals use the Tscherne Classification of Soft Tissue Injury, described in a study published in the journal Colombia Médica in 2014, among other publications. Also, for example, a tibia fracture and injuries related to its soft tissue are exemplified.

According to the Cerne classification, soft tissue damage is classified according to the following grades:

Grade 0
Grade 0 occurs in minor external force fractures, such as tibial vertebral fractures in skiers. There are no obvious clinical signs of injury.

Grade I
Injuries in this group appear when moderate external force fractures occur. An example of a Grade 1 injury is an ankle fracture dislocation. The released external force produces mild to moderate soft tissue bruises.

Soft tissue damage can also be caused by the pressure exerted by the fracture fragment.

Grade II
Class 2 injuries occur in high external force mechanisms. An example of this type of soft tissue injury is a partially closed tibia fracture due to a traffic accident.

In this case, the soft part absorbs a lot of external force during the impact, resulting in a deep bruise. This category also includes impending compartment syndrome.

Grade III
Finally, there is a Class 3 injury. These injuries are associated with serious skin and muscle damage, such as:

Damage to blood vessels.
Acute compartment syndrome. This is a serious condition associated with increased pressure in the muscle compartment. This can lead to nerve and muscle damage as well as blood flow problems.
Crush damage.
Moulting damage.
More fissile injuries belong to the most severe group of soft tissue injuries.

The classification system has changed to include more elements and aspects that medical professionals need to evaluate.

Cerne's classification has recently been expanded. In this expanded classification, a 5-point scale is used that independently quantifies the severity of skin, muscle tendon, and neurovascular injury.

This classification describes moulting injury more accurately than the previous classification. However, most authors still use Cerne's classification.

Although classifying soft tissue damage can actually help, its severity continues to increase. Therefore, it is often difficult to reliably classify specific injuries.

Treatment of moulting injuries
Soft tissue recognition and management are the two most important aspects of treatment. Of course, in this case, expert help is essential and urgent.

According to the previously mentioned classification, Grade 2 and Grade 3 injuries are generally better healed with temporary external fixation and delayed surgery.

Moulting injuries in this regard almost always require severe surgical intervention. In addition, a study published in the Journal of Emergencies, Trauma, and Shock highlights the importance of an interdisciplinary approach. Some treatment options are:

Reimplantation: This consists of joining the amputated body parts together. Like the fingers of a hand, for example.
Blood vessel regeneration: restores the blood supply to the area suffering from a lack of blood supply.
If not possible, the medical professional will perform skin graft surgery or flap surgery. However, it is sometimes impossible to preserve the limbs. Therefore, the medical professional must perform the amputation.
