There are some addresses sending bytes automatically to steal referrals, even if you sent the address privately, it's at the network level. I guess they detect the transactions of those amounts and then send them the minimum and take the referral.

The system should work to take the first one. I know it is complex, but this is just rewarding spammers and bots.

Hard to say. Generally, it should be the first one, if some input has more closer amount to what is needed then it probably that, but if both are exactly what is needed (49000 + 588) then maybe it's the first again. Haven't tried.

I was thinking about it more and I think I know why it is difficult to reproduce. It might be that they are monitoring certain addresses that are known for sending lot of funds to new addresses, so they are not monitoring all transactions and they might not even run a full wallet. Basically they are following the transactions of most popular Steemians whose posts are most popular and who get asked the most to send the fee.
