I've noticed that its mostly public hospitals that smell. I talk based on my experience in Nigerian hospitals. The reason may be due to poor maintenance of the public hospitals and the laissez faire attitude of the cleaning staff.
Poor maintenance might be part of it, but a major part of it is the crowd flow.

Exactly. The prevalence is higher in public hospitals due to poor management, high patient load etcz.
Note that, well-coordinated hospitals do know how to manage the smell.
A clean, disinfected atmosphere is the best place to handle health.
