Case 1

Richard is a 15-year-old who has been having poor grades at school despite being very attentive and hard working at the school.

His mother thinks its because he is teased at school because his breasts look like that of a girl. Richards father, however, is not in agreement with this flimsy excuse.

The father decides to take Richard to see a doctor when the doctor finally examines him, he finds out that he actually has small round firm testes.

  • What do you think is Richard's medical problem (diagnosis)?
  • If you were a doctor, how would you solve the problem?
  • What lessons have you learned from this story?

Case 2

Favor is an 18-year-old who has been having recurrent headaches distorted vision for the past 5 years. In fact, she remembers that this problem actually began when she started seeing her menses.

Her friends have been laughing at her since this occurred in her life because she did not actually seem to have the right shoe size for her age because her feet were always bigger than others of her age.

Her mother decides its high time favor consulted a doctor to really understand what the problem is with her only daughter.

Tell us about yourself

I am Blessing Ogbonnaya. I am a Dentist by profession, a Love and Relationships Coach by passion, a graphics designer in the making, a mother to a wonderful son and a wife to an amazing man. I am also one of the co-founders of AIR-CLINIC.

I am popularly known as thelovejunkie on almost all my social media platforms. I am a writer and have been blogging way back since 2014 but then Steemit gave me value for my work and articles by monetizing.

I started an NGO in 2014 called #ProjectCompassion where we reach out to the less privileged of the society and put smiles on their faces and Jesus in their hearts. I also run a Relationship School biannually where myself and Husband together with special guests teach people on matters of Love, Relationships, Marriage and everything in between and thankfully, God has been faithful in all of these.


WHY DO YOU LOVE AIR-CLINIC?

Apart from Air clinic being like a baby to me, I love airclinic for its uniqueness and consistency over the time on the blockchain. I say that because I have seen quite a number of health blogs which started off here on the blockchain but have not been able to level up with AIR-CLINIC. Indeed, we are a force to be reckoned with and we are changing the face of healthcare through the blockchain.


HIGH AND LOW POINTS ON STEEMIT

For my high point I'd say it was when I newly joined steemit.com that was towards the end of Oct last year also for getting to know and meet people with all the meetups and all, it was really fun and don't forget the market was really good so it made such sense blogging and getting something tangible.

My low point on Steemit is now, the market isn't really encouraging and it has made so many have stopped to post on their blogs also, Steemit is like the real world, nothing goes for nothing.


HOW DO YOU BALANCE YOUR STEEMIT LIFE WITH REAL LIFE

Oh I balance them just well, Steemit life is a virtual life and that's how I see it. The real life is the main thing and it's the most important. Being on Steemit has been good and fun but then, having to reach out to people and letting them read your work on matters of Love and Relationships and see people get blessed is just it for me and not other necessarily the money. So far so good, Steemit has been a blessing.


MOTIVATION TO STUDY YOUR CAREER

Motivation came from first me being a caring person wanting to see people happy and healthy and then also from my family having tooth issues.


CHALLENGES/SURVIVAL STORY IN YOUR SPECIALTY

I remember back then when I had admission into school, I initially wanted to read medicine and surgery but I later changed my mind and went to dentistry because few people were in that field and thank God I got it.

My challenge/survival story I would say is by the God factor, God brought me this far. He brought me through med school, house-job, service years etc. I really love what I do as a dentist because we are 3 professionals in one (doctors, artist, and engineers) because we improve humans oral health


FINAL WORDS OF ENCOURAGEMENT TO THE AIR-CLINIC FAMILY

Well my advice for doctors out there is First, you all need to be part and parcel of what Air clinic is doing because we are about taking over the health system in a good way.

Also, I'd say as doctors, we should leave indelible marks on people's heart, don't just treat them, minister to them and let them become better all round because of you.

Case 1

My initial diagnosis is:Gynaecomastia with testicular tumor
Let us have a short discussion:
Enlargement of the male breast is known as gynaecomastia which happens due to wide varieties of causes. But the main pathology is an increased amount of estrogen (one of the two female hormones) over androgen in male which contributes to breast tissue development. Puberty is one of the physiological causes but that is not the case here although Richard is in pubertal age. Read more about gynaecomstia here: http://bit.ly/2zOjQHN

He has firm testicular mass which indicates testicular tumors. Seminomas and testicular teratoma are two commonly occurred tumors in the testis. Seminomas are rarely associated with raised hCG (human chorionic gonadotrophin hormone) but in 80% of cases of teratoma, hCG is positive. hCG has some estrogenic effect which contributes breast enlargement. Another point is, teratoma happens in earlier ages than seminomas. Read more about testicular tumors here: http://bit.ly/2zP5LJZ

So considering all the factors, my final diagnosis is
Gynaecomastia with testicular teratoma

Investigations:

  • USG of the testes (scrotum)
  • Urinary pregnancy test for Beta-hCG
  • Blood tumor markers: Beta-hCG and AFP (alpha-fetoprotein)

Case 2

My diagnosis is acromegaly due to excess growth hormone(GH) due to GH producing tumor in pituitary gland. Simply, Pituitary tumor with acromegaly. The case has mentioned that Favor started having the problem when started menstruating but what happened later, hasn't told us. Beside enlarged leg size, if she is not having menstruation(amenorrhoea), then the chance of prolactin-secreting tumor known as prolactinoma of the pituitary gland also there. She is also having a visual problem which is due to the mass effect of pituitary tumor on the optic chiasma (optic chiasma is just situated above the pituitary gland).

Final diagnosis: Pituitary tumor (either GH producing tumor or prolactinoma or both) with mass effect with acromegaly

Investigation:

  • Measurement of blood GH level
  • OGTT (Oral glucose tolerance test)
  • Measurement of IGF-1 (insulin-like growth factor-1)
  • Pituitary MRI (MRI of the brain)
  • Visual field test: Bitemporal hemianopia (Upper quadrant defect > lower quadrant defect= Pituitary tumor)

Seems like my diagnosis was correct.
