Always seems to be a massive and unexpected catastrophe with each HF.
I don't have any direct involvement with the C coding for the blockchain, but as I understand it - despite a lot of testing this time - there were scenarios that weren't accounted for. This is part of why the software companies that support investment banks with billions/trillions of dollars of assets will employ large teams of well paid testers - we don't have that for Steem at this point so bugs slip through.
