I don't think anyone can fail as horribly as Mt.Gox did. The CEO of Gox basically knew what was going on, and lied to everyone and shamed himself in Japan.

Those are pretty big shoes to fill. Most people just get hacked and go bankrupt like Cryptsy. Even Cryptsy did a better job at failing than Mt.Gox.
