Have not observed packet loss or random disconnects, this of course can depend a lot on the electrical installation and the devices plugged into it... there could be some electrical appliance that is causing the issues.
D-Link have a simple tool for management of their PowerLine adapters over the network, so if you have remote access to a PC in the location that is not connected to a PowerLine adapter you might be able to reconnect the adapters remotely.
Not sure what brand of devices you are using as well as what generation are they though…
yes we have older no-name power adapter. there's no lan socket and customers visit so a lan cable would be ugly.
it works but not very satisfied overall, the best choice in my location would be extra flat cable but the carpet is glued to the ground.

I wasn't very happy with some earlier generation devices, but it seems that the latest generation PowerLine adapters are behaving pretty good... :)
