That's because of inconsistencies in the returned values of the account history (Steem node related). I reported this recently here on GitHub and I hope they will finally fix that.
It's a known bug since Oct 27, 2018 and in my eyes it's a major one, because exchanges might have problems with uniquely identifying single operations within one transaction.
