Obamacare is the Insurance Company Relief Act not the Affordable Care Act.

It must be repealed.

Today, I became a supporter of expanded Medicaid in the U.S. Without means testing.

Today, is the day I realized that there is no going back to health care costs that I can afford to pay out of pocket like my father did. Sadness comes because I know the system has been stacked against us. Like the system confronting the people of Boston just before the Tea Party... Just before the revolution. There is no place to go though.

Obamacare will continue to cause "Insurance" costs to rise forever. Until we are all broke.

It's not insurance if you PLAN to use it,

so we need Public Health Care that allows for private insurers to continue to sell insurance to healty people who qualify for insurance; to "Top Up" if you will.

But for regular health maintenance, every-day doctor visits for the chronic maintenance we all need, we have to cutout the middlemen who take our premiums for his profits because the actuarial tables have been skewed in his favor by all the high dollar maintenance cases out there.

We the people CAN fund an Expanded Medicaid System on the Countywide level, and eventually ask for Federal Grants, and keep the private system with all the choices we love to have. To create competition between public and private systems.

Here are some of my suggestions on taxation and healthcare:

Create a Countywide Payroll Tax. Tax everyone's paycheck on a countywide level at 3% of his net earnings. This is not an income tax its to be called "Countywide Medicaid" to include dental and optical. If you have a Job as an hourly or salaried "Employee" then you will be subject to the Health Care Tax. Think of the County Wide Sales Tax we currently have funding Jackson Memorial Hospital in Miami - Expand that to a flat 3% Payroll Tax and offer non-profit health care services for all residents - Period." This should provide a minimum level of services for chronic needs which provides the competition that the Private Sector Needs to keep its fees for topping up, reasonable.

These taxes should not affect the employer as they are payroll taxes, taken at point of payment and are not based on the income of the company. Employers don't get sick... people do. Ergo people should make the payments not corporations.

This will cover the expansion and will even cover the unemployed and children. THIS IS IMPORTANT, however,

County Wide Medicaid should be restricted to public hospitals and clinics and only for County-Wide Residents .

Payroll deductions and possibly doc stamps has to fund it at 100% or zero sum with possible federal grants coming later. So the percentages may need to be adjusted. Caveat! Countywide Medicare must be restricted and invalid at privately held hospitals, clinics or doctors. All Medicare and Medicaid funded care is only through government operated (county) hospitals or clinics.

The law should require that the county employ 1 doctor per 500 residents, and 1 nurse per 250 residents a minimum of 40 hours per week, and have a minimum of 1 hospital bed for every 1750 residents; made up of specializations as determined by each county's board of health.

Each procedure paid for by Medicare must be capped at a zero profit to prevent privatization of county hospitals. This will put some real pressure on private insurers and hospitals to lower their fees and create competition with County Wide Medicare. The restriction of Medicare patients to only receive services at government owned hospitals is the incentive for the private hospitals to compete to provide better services than is provided in the public system at a really reasonable cost. (This will also end uninsured costs to private hospitals and put pressure on public hospitals to provide decent care.)

Public employees should be exempt from liability in accordance with state statutes covering all public employees and government agencies based on qualified immunity for public employees.

Counties with close proximity to other large population centers, similar to Miami, Ft. Lauderdale, and West Palm Beach, should all consider enacting the same laws at the same time and consider sharing public resources so that people that migrate in or out of the County due to the creation of this law cannot escape the payroll taxes and must be based on the location of the employer not the address of the employee.
