Two Lessons Hospitals Can Learn From Their COVID Response
Although hospitals and healthcare systems prepare plans for disasters, such as tsunamis or earthquakes, the onset of COVID last spring revealed many were ill-prepared to mobilize in response to a global pandemic, particularly in the allocation of resources and provider supports. Two vexing challenges, in particular, have emerged: implementing an effective incident command center designed to orchestrate a system-wide, coordinated response to the pandemic; and second, providing just-in-time learning for providers to ensure they have the training, knowledge, and skills necessary to ensure caretaker safety and optimal patient care.
As every hospital administrator knows, the cornerstone of hospitals' emergency planning is the Hospital Incident Command Structure (HICS), which exercises additional oversight to see the hospital through the crisis. With varying degrees of success, hospitals have established this command structure to oversee all operations and deploy resources to address the current crisis. In most cases, the implementation of HICS is a temporary state, after which the tendency is to resume normal operations.
With COVID, we saw the HICS culture evolve rather quickly, and we learned there are aspects of this culture that we should view as positive enduring characteristics. These include a clear line of decision-making authority, vastly improved communication (via daily huddles), a willingness to disseminate and implement updated guidelines, and far greater transparency and accountability.
