How are you all?In previous lesson I explained shoulder abduction but, in today's post I will share shoulder horizontal abduction with you.
•Basically shoulder abduction is of two types:
•Shoulder horizontal abduction.
•Shoulder vertical abduction.
In today's post, I will share shoulder horizontal abduction with you.To check the strength of muscles that are involved in shoulder horizontal abduction, first you have to know that what is shoulder horizontal abduction.

Shoulder horizontal abduction:

Shoulder horizontal abduction is defined as movement of shoulder away from the body in horizontal direction.

Muscles involved in shoulder horizontal abduction:

•Muscles that are involved in the shoulder horizontal abduction are:
•Deltoid(posterior fibres).
•Deltoid(Middle fibres).
•Infraspinatus.
•Teres minor.
•Rhomboid major.
•Trapezius(Middle).

Range of shoulder horizontal abduction:

•0°-90°(range,90°)when starting from a position of 90° of forward flexion.
•-40°-90°(range,130°)when starting with the arm in full horizontal adduction.

Grading system:

•We use grading system for checking the strength of muscles.Here we will apply the grading system for checking the strength of muscles that are involved in shoulder horizontal abduction.
•5 grade shows normal muscle strength.
•4 grade shows good muscle strength.
•3 grade shows fair muscle strength.
•2 grade shows poor muscle strength.
•1 grade shows trace activity of muscle.
•0 grade shows no activity of muscle.

Patient position:

•Position of patient should be prone lying.Shoulder abducted to 90° and forearm off edge of table with elbow straight.

Therapist position:

Therapist will stand at the test side.Ask the patient to lift the elbow up toward the ceiling.Therapist hand giving resistance is contoured over posterior arm just above the elbow.

Test:

•Patient horizontally abducts the shoulder.Care should be provided to not allow the humerus to drop(lower), allowing the substitutions of the rhomboids.

Instructions to patient:

•Lift your arm up towards the ceiling.Hold it.Don't let me push it down.

Grade 5:

•In grade 5, patient position should be prone lying.
•Therapist hand used for resistance will be placed on the distal end of humerus.
•Full resistance will be applied by therapist at the distal end of humerus in grade 5.

Grade 4:

•In grade 4, again patient position will be prone lying.
•Most major difference in grade 4 and 5 is that in grade 4, moderate resistance will be applied by therapist and in grade 5, maximum resistance will be applied by therapist.
•The resistance in grade 4 and 5 will be applied at the distal end of humerus by the therapist.

Grade 3:

•In grade 3, Patient position should be prone lying with the elbow flexed.During grade 3, patient should be in gravity minimized position.

•Patient performs this movement without any assistance or resistance of the therapist.

Grade 2:

•In grade 2, Patient position should be short sitting.Therapist will stand at the test side.Therapist support the forearm under volar aspect and palpate over the posterior surface of the shoulder just superior to the axilla.

Test in grade 2:

•Patient attempts to horizontally abduct the shoulder.Be careful not to allow the patient to drop the arm,substituting the rhomboids.

Instructions to patient in grade 2:

•Try to move your arm backward.

Grade 1:

•In grade 1, patient position should be sitting.Therapist will stand behind the patient.Stabilize by contouring one hand over the superior aspect of the shoulder and the other one over the scapula.Palpate the fibres of posterior deltoid below and lateral to the spine of the scapula and on the posterior aspect of the proximal arm adjacent to the axilla.

Test in grade 1:

•Patient slides the arm across the table in horizontal abduction.
•Palpable contraction will occurs with no motion.

Grade 0:

In grade 0, there is no discernable activity of muscles occurs.
