The diary was actually with Lucius Malfoy and was passed to Ginny in her cauldron. Harry Potter however found it in the girl's bathroom.

Gaunt Shack ,Dumbledore destroyed it in his office with Gryffindor's sword
✅ Salazar Slytherin’s Locket:
In a cave with lake ,guarded by Inferi found by Harry Potter and Dumbledore
✅ Helga Hufflepuff's Cup:
Gringott's - the wizard bank (Vault owned by Lestrange)
✅ Harry Potter himself
wasn't an intended hocrux but became one when Lord Voldemort gave away some part of his soul to Harry when his spell sprung back.
✅ Nagini:
In the possession of Lord Voldemort
