I was actually searching for some community so which are according to my knowledge and where I can share some information articles as well so when read this contest I am extremely happy actually to participate in this contest for the very first time I am participating in this community and I hope my article will help you and increase your knowledge how to take vitamin B12 and when to take and what is the best way to take vitamin B12

I am doctor Iqra and I have completed my MBBS and after that I have done my house job and now I am working as medical officer. So basically I am working in medicine so todays is topic is one of my favourite topic to talk about.


๐Ÿ’‰Which of these three routes of medication administration do you consider the fastest and most effective to relieve pain, why?

So this topic is actually very vast and it is very hard to cover all the answers in one line so I will try to explain it a little bit as a doctor I will also give suggestions on what is the best route and what is the most commonly used route for administration.

Fastest Route

So I will cut down the question into part the first part is what is the fastest route so it's very obvious that the most fastest way for pain management is intravenous because we all know that intravenous is directly entering into the vein@(blood)so it has earlier effect rather than taking oral medication.

As an example I can give you for acute when management there are a lot of drugs that we give infra Venous and that has really good effects for example opioids arw most commonly used morphine are most commonly used ketamine has really good effects so basically we can see that if the pain is very sharp in nature and its continuous in nature and the patient can't bear it so the most fastest route to relieve the pain is that's bit all important intravenous and this is the fastest way for pain management.

Three Fastest Route

Another part of the question is what are the three fastest route for for pain management I have talk about the most fastest way for acute pain management that was intravenous and if we talk about other ways so there a number of list of different ways to manage the pain it's

  • Intra muscular
    or
  • Subcutaneous route

This is also very commonly used in our hospital practice and because most of the patient don't want to have medication by intravenous so we preferably give intra muscular or subcutaneous.

For example after many surgeries of the patient is having surgery and after that he is having pain so he doesn't want to actually have intravenous medication again so we prefer to give him subcutaneous or intra muscular.

So these three are the fastest route of drug administration for pain management intravenous intra muscular and subcutaneous and if we talk about the other ways there is also other common way which we give to the patient is orall because the most of the time patient are very afraid of the syringe ๐Ÿ’‰ so other most common route is orall.

  • Transdermal Route
  • Trans mucosal Route
  • Intra nasal Route
  • Sublingual Route
  • Buccal Route
  • Epidural Route
  • Peripheral blocks

So there are a list of ways we can administrator the drug for pain management but as I already mentioned these are also helpful for pain management but the fastest way is intra muscular and sub cutaneous. And most commonly used is oral.

๐Ÿ’Š There are two ways to administer injected treatment, intravenous and intramuscular. Which of them do you prefer in case you need quick relief or discomfort?

So the intravenous and intra muscular these both are most commonly used in hospital practice and for giving patient certain type of medications multivitamins and other things which directly explain entering into blood circulation so if I talk about intravenous this is basically most commonly used for giving patient of fluid and which is directly enter the way and this is used for the fastest absorption and potency is very high so when the patient is in shock or dehydration or certain other medical conditions and so if we want to give the patient most immediate medication so yes the intravenous is most fastest way of administrating the medication into your blood circulation for getting immediate response

WHEN I WAS SEVERELY DEPHYADTED THE FASTES ROUTE IS IV

I talk about the intra muscular so there are other indications of using intra muscular way firstly I will explain that intra muscular route is also faster but it is slower than intravenous because in intravenous that medication is directly going into the blood circulation and intra muscular is first entering into the muscle and certain layers and then entering into the blood circulation so intra muscular way is slower way but it is effective and we most commonly used in our clinical practice and secondly that is having certain other indication for example for example and certain antibiotics vaccines and for acute pain management the intra muscular route is also very good.

So if I give my clinical practice suggestion I see more than 100 OPD patient daily so what I prefer is that was of the patient are very afraid of intravenous root no doubt it is the fastest way but if I have to give any fluid or patient is in critical condition I prefer to give intravenous root no matter patient is agree but my clinical practice the most common route for me to use is intra muscular.

Then offcourse oral so intra muscular for any antibiotic use I use most of the time the intra muscular way nn our clinical practice intra muscular route is more common rather than intravenous but in hospital practice intravenous is more common than inter muscular because whenever you are entering into the medical emergency the first thing to do is getting the intravenous catheter so that was my experience that intra muscular is more commonly used than intravenous but intravenous is most fastest way.

๐Ÿ’‰There is a very good medicine for our body vitamin B12, why do you think they indicate it more in injection and not in pills? Do you think they do not have the same components or the same effect?

So vitamin B12 is most commonly used in our clinical practice for me I recommend a lot because in our part of country in Pakistan I think a lot of people have diabetes so when the patient have diabetes then nerve become weaker with the passage of time so I recommend most of the time the patient vitamin B12 and I recommend actually injections more because it has fastest absorption rather than orally give now I elaborate how to differentiate between tablet and injection.

So vitamin B12 actually give if the patient have anaemia are patient is is easily become tired or fatigue so we usually give vitamin B12 because it is very good to increase your oxygen capacity to muscle , organs and that will automatically very good for your red blood cell include your brain as well.

So actually a research which was occur in 2008 so what they did actually only 1.3% of the people who given oral medication where as absorption rate was 55% the patient who given injection of vitamin B12 it means that vitamin B12 injection are more fastest absorb because most of the time they are administered muscular and so it is directly enter into your blood stream where as if we give oral tablets this firstly enter into your digestive system so it will take time for absorption and that's why it has poor rate of absorption.

If I talk about my clinical practice so I think the daily vitamin B12 has really good effects and it is more effective rather than taking the monthly injection of it because most of the time you really and not go to a good doctor so over the site of injection is inflammation it is not injected in a good way so for me is good option rather than injection form.

And if I answer to the question that vitamin B12 pills and injection are the having same component and effects no the frequency is different for example the injection form is given 1000mcg monthly for at least 4 to 8 weeks and which is entirely different from oral dosage.

If I talk about and the components of vitamin B12 injection so I will mention what it is containing

As from name it is clear that it is having cyanocobalamine which is actually vitamin B12 and main component is B12.

  • Benzyl alcohol
  • Citric acid
  • Sodium citrate
  • Ammonium sulphate

And also having sodium chloride so I am not mentioning how much percentage it is containing but this is the main component of vitamin B12 injection form.
Whereas tablet mainly containing active form of vitamin B12 that's the reason it takes time to absorb.

๐Ÿ’ŠIf you are one of those who do not take pills and you do not like injections, what other method are you looking for to relieve pain or discomfort!?

If I talk about like I am a doctor and most of the time when I am prescribing anything to the patients so I have also headache or fever and but I don't want to take tablets or I don't want injection so what I do is actually advice for others as well firstly it depends upon what type of pain you have and where you have paint and what is the nature of the pain then according to I can prescribe what to do but if the person is not taking tablets or not taking injection so what to do in this situation.

So there are a many ways actually to relieve pain firstly do

  • Exercise
  • Physiotherapy

Physiotherapy is also a very good way of relieving pain and they give you different type of exercise different kind of massage with oiling and different techniques to relieve your pain without using any tablets or injection so physiotherapy is one of the best way to relieve pain most of the time when patient came to me and I don't have any solution like injection or tablet form so I recommend them to go to a good physiotherapist.

  • You can do meditation as well to relieve your pain

  • And there are also some other techniques Chinese technique of acupuncture.

  • And sometimes the cold or heat therapy is also very suitable and people have firm believe in giving cold or heat therapy is also very helpful in relieving pain.
