Today I will deal with a topic quite named worldwide, ACNE.

lesion or disease?

Acne is considered a chronic and inflammatory skin disease, caused by various skin lesions such as comedones, papules, nodules and scars.

Acne is a widespread disease worldwide and affects numerous individuals in the population, mainly those between the ages of 12 and 24.

85% of adolescents are affected by this condition and many of them reach adulthood still suffering from it.

Nowadays, adult acne is becoming more and more common, and many patients over 20 years of age also come to appointments worried because they had never suffered from this disease in adolescence and now have outbreaks.


Elementary skin lesions present in this disease

Elementary lesionsDescriptions
PustulesSmall cavities of the skin with pus inside, usually appearing around the cheeks and nose.
PapulesSolid, reddish elevations of the skin less than 1 cm in diameter, with no central point.
ComedonesThese are blackheads (open comedones) or "blackheads" (closed comedones).

The scars, nodules and cysts listed below are the result of all the lesions together.


Etiology of acne

Acne can appear for various causes but the most prominent are:

🌼 Sebum production and hyperplasia (enlargement) of the sebaceous glands.

The sebaceous glands are responsible for generating the oil needed to keep the skin soft and smooth, they are located in the dermis (second layer of the skin)

🌼 Abnormal desquamation of keratinocytes.

Keratinocytes are the cells that make up most of the epidermis (the outermost layer of the skin).

🌼 Presence of propionibacterium acnes, the bacterium that causes acne.

🌼 Hormonal activity, such as menstrual cycles

🌼 Stress, caused by the adrenal glands.

The adrenal glands are a small gland that produces epinephrine, steroid, and norepinephrine hormones. These hormones help control heartbeat, blood pressure, and other important body functions.

🌼The use of some medications such as contraceptives, steroids, among others.

🌼 Exposure to high levels of chlorine compounds.


Acne according to the degree of lesion

According to the degree of lesion acne is divided into 3 degrees:

🌼 Moderate acne: There is presence of numerous papules, pustules and comedones (between 10-40). They may appear on the face and trunk.

🌼 Moderately severe acne: The presence of papules and pustules is greater (40-100), there are also deep, inflitrating nodular lesions. The affected areas are not only on the face and torso but also extend to the back.

🌼Severe acne: These are the strongest types of acne of all, they must be treated by the dermatologist with topical and/or oral medications, they are known as nodulocystic acne and acne conglobata.

Patients present with many large and painful nodular lesions, pustular lesions along with many small papules, pustules and comedones.


Types of acne

There are many types of acne according to their cause and severity

🌼 Acne vulgaris: It is the most typical in adolescents, this according to the number of lesions varies in severity, it is common to observe: papules, pustules, cysts and nodules on the face, neck and upper shoulders and trunk.

🌼Comedonal or comedonal acne: Only the seborrhea of the skin (greasy) appears with numerous comedones. This is the mildest type of acne.

🌼Papular acne: This is the acne in which more blackheads than closed comedones are observed, lesions with pus are almost not seen in this type of acne.

🌼Atrophic acne: It is a severe papular acne that causes keloids and scars on the skin.

🌼 Keloid acne: It is composed of scars that thicken forming keloids. Dark-skinned people are the most prone to this disease.

🌼Premestrual acne: This is composed of lesions that appear or worsen during the period prior to menstruation, due to hormonal changes that occur during this process.

🌼Tropical acne: This is the one that affects most of us who live in hot and humid areas, it is predominant in areas such as the back, buttocks and thighs.

🌼Contact Acne: It is produced by voluntary or involuntary contact with substances that favor the production of comedones. Some make-up, sunscreens, creams, exposure to industrial sectors of hydrocarbons, petroleum oil or resins are examples of this type of case.

🌼Iatrogenic acne: is caused by the side effects of certain drugs, such as corticosteroids.

🌼Acne conglobata: As mentioned above, one of the most severe forms of acne, it presents all the lesions together and aggravated. It may be associated with other immunological diseases.

🌼Cystic acne:The most severe of all types of acne, with the presence of comedones, pustules, papules and especially inflammatory nodules, formation of painful abscesses and cysts with occasional bloody crusts.


As a result of the pandemic, a new one was added to this group: MASKNE

The maskne is a new acne caused by the use of masks, this is a type of acne caused by the friction generated by the masks on our skin. When we talk or breathe, hot air is trapped in the mask, generating an environment suitable for the growth of bacteria and skin mites, the skin breathes less and that causes sweating, in addition to the obstruction of the same.

My advice to avoid this acne is to keep as clean as possible your mask, do not prolong its use for more than 6 hours and if so change it constantly after that time, maintain good hygiene of the face and avoid saturating it with makeup or unnecessary products.


Recommendations for patients with acne

  • Facial cleansing with special cleansers (Cerave, Neutrogena, Cetaphil, Dove are some of the brands where you can get a good anti-acne line).

  • REMOVE MAKE-UP

  • Shampoo your hair regularly, especially if you have an oily scalp.

  • Keep your hair away from your face, avoid wearing hats or bandanas that can cause infection in the affected area.

  • DO NOT squeeze, scratch or rub pimples.

  • Avoid greasy creams and cosmetics.
