Hola a toda la comunidad, como miembro del equipo de salud, estoy atenta a cualquier urgencia que pueda presentarse, el día de ayer llamó un paciente informando que tenía dolor espontáneo en un diente que no lo dejó dormir, y quería que por favor le diera un cita para poder verlo y realizar el procedimiento necesario para aliviar el dolor.

Hello to all the community, as a member of the health team, I am attentive to any emergency that may arise, yesterday a patient called informing that he had spontaneous pain in a tooth that did not let him sleep, and wanted me to please give him an appointment to see him and perform the necessary procedure to relieve the pain.

Al examen clínico se observa el premolar superior derecho con caries profunda, inflamación, dolor a la percusión tanto vertical, como horizontal, y debido a las condiciones que no podemos realizar tratamiento muy largo se decidío realizar la extracción del premolar.

On clinical examination the upper right premolar was observed to have deep caries, inflammation, pain on percussion both vertically and horizontally, and due to the conditions that we cannot perform very long treatment it was decided to perform the extraction of the premolar.

Comenzamos con la colocación de anestesia infiltrativa, se aplica lidocaína al 2% tanto en vestibular como palatino, se espera que sienta el efecto anestésico para proceder a realizar la sindemostomía y luxación con el elevador recto.

We begin with the placement of infiltrative anesthesia, lidocaine is applied at 2% in both vestibular and palatine, it is expected that you feel the anesthetic effect to proceed to perform the syndemostomy and dislocation with the straight elevator.

En este caso, el diente no tine vecinos y debe realizarse con sumo cuidado, al lograr romper los ligamentos periodontales y sentir la movilidad, se toma el forceps #150 indicado para ese diente se hacen movimientos hacia vestibular y de rotación para desalojarlo del alvéolo de esa manera el diente sale sin ninguna complicación.

In this case, the tooth does not have neighbors and must be done with great care, to achieve break the periodontal ligaments and feel the mobility, take the forceps # 150 indicated for that tooth are made movements towards vestibular and rotation to dislodge the alveolus in that way the tooth comes out without any complications.

Para finalizar, cureteamos el alveolo para limpiarlo de cualquier resto infeccioso, espícula de hueso o resto de corona que pudo haber caido dentro y nos dificulte la cicatrizacion, se lava con solución fisiológica y se coloca el tapon hemostático.

Finally, we cure the alveolus to clean it of any infectious remains, bone spicules or crown remains that may have fallen in and made healing difficult. The alveolus is then washed with physiological solution and the haemostatic plug is placed.

Se le dan las indicaciones necesarias para un buen post operatorio:
Tratamiento antibíotico y analgésico,
Dieta líquida por 24 hora
Dieta blanda por las siguientes 36 horas
Tomar alimentos y bebidas frías
No escupir ni hacer buches
No molestarse la zona con la lengua
No tomar bebidas con pitillo
No tomar lacteos.
Domir semisentado.

You are given the necessary indications for a good post-operation:

• Anti-biotic and analgesic treatment,
• Liquid diet for 24 hours
• Soft diet for the next 36 hours
• Eating and drinking cold foods and beverages
• No spitting or swallowing
• Do not bother the area with your tongue
• Do not drink drinks with a cigarette
• No dairy.
• Sleep semi-seated.

Siguiendo éstas sencillas instrucciones tendremos un post operatorio satisfactorio sin ninguna complicación, teniendo en cuenta las circunstancias de cuarentena que vivimos.

Following these simple instructions we will have a satisfactory post-operative without any complications, taking into account the quarantine circumstances we live in.
