Hola a toda la comunidad, cuando la enfermedad periodontal es crónica y el paciente por pena o temor no asiste a la consulta, el pronóstico es malo, la movilidad es muy notoria y no tenemos muchas opciones que presentarle al paciente.

En este casos en, particular el paciente refiere dolor y dificultad para ingerir alimentos por la movilidad que presenta, no puede masticar los alimentos, al examen clínico se observa movilidad por enfermedad peroiodontal ademas de perdida de los tejidos de soporte y se indica exodoncias de todos los dientes afectados, y elaboración de prótesis removible.

In this case in particular, the patient refers pain and difficulty in eating food due to the mobility he presents, he cannot chew the food, the clinical examination shows mobility due to periodontal disease, as well as loss of support tissues and indicates exodontia of all the affected teeth, and elaboration of removable prosthesis.

Se coloca anestesia infiltrativa en todos los dientes, como tienen mucha movilidad el paso de la luxación no es necesario, y se va directo a la tracción con el forceps, por ser dientes unirradiculares, se hacen movimiento de rotación para desprenderlo de la poca inserción que tienen.

Infiltrative anesthesia is placed on all the teeth, as they have a lot of mobility the dislocation step is not necessary, and it goes directly to the traction with the forceps, as they are uniradicular teeth, they make a rotation movement to detach it from the little insertion they have.

Se curetean y se lavan los alvéolos con solución fisiológica, se coloca tapón hemostático y se le indican las recomendaciones post operarorias:

Tratamiento antibíotico y analgésico,
Dieta líquida por 24 hora
Dieta blanda por las siguientes 36 horas
Tomar alimentos y bebidas frías
No escupir ni hacer buches
No molestarse la zona con la lengua
No tomar bebidas con pitillo
No fumar

The alveoli are curetted and washed with physiological solution, a haemostatic plug is placed and post-op recommendations are given:

• Anti-biotic and analgesic treatment,
• Liquid diet for 24 hours
• Soft diet for the next 36 hours
• Eating and drinking cold foods and beverages
• No spitting or swallowing
• Do not bother the area with your tongue
• Do not drink drinks with a cigarette
• No smoking

Esperamos de 15 días a un mes que el proceso de cicatrización este avanzado y el hueso se reabsoba para tomar impresión, diseñar y elaborar la prótesis.

We wait from 15 days to one month for the healing process to be advanced and the bone to be reabsorbed in order to take an impression, design and make the prosthesis.
