Here's my most recent chapter. Obviously, this is mostly just a style question, but I think having your writing as foremost is important.

So, at the top, I put the image.
Then, I do [ previously... ](link of last chapter)
Then I go on to the chapter with # and ## for the chapter number and location.
So, where you have your chapter number and title, I'd use # or ## just before it (with a space) to bring attention to the chapter beginning.
Whereas with another type of article, I don't mind putting image credits with the image, in this case, I'd probably put them at the end, but again, that's just me.
I mostly want to give you another tool to play around with and see what you prefer.

Totally up to you. It used to be that you couldn't change it at all after 7 days, but you can change it indefinitely, I think. The only thing you can't do is delete it altogether.
I actually use Steempeak for almost everything on Steem because I can have templates for my posts - which means I can play there and see what it's doing, then just save it for future use. (If you haven't already tried it, it's really simple - replace "steemit" for "steempeak" - and use KeyChain to log in.)
I use Steempeak now for my series templates and others.
