Slipped capital femoral epiphyis (SCFE)

You have probably never heard of this disease, because in the field of orthopaedic surgery and traumatology it is not a common pathology. It is a kind of hip fracture that occurs in children.

Since the bone has not yet fully developed, there are some formations at the joint level called Growth Nuclei.

Slipped capital femoral epiphyis is precisely a lesion in the hip growth nucleus, and precisely because it is an area of important movement for our locomotor system, treatment must be urgently rapid or emergency.

Many of these cases are associated with trauma, as well as other cases of endocrine-based disease, so it is always appropriate to perform some laboratory tests to diagnose other diseases early.

The patient's story

Manuel is a 15 year old boy, one of his favorite sports is baseball, since he was 10 years old he has practiced it and dreams of one day playing as a professional in some of the country's famous teams.

A week ago he was preparing for a big game and after a hard day of practice he went with his friends to play near the baseball field. Surprisingly, after sliding on some stones, he falls on an uneven ground that causes a strong pain in his right hip.

Immediately his friends call Manuel's parents and get him out together to take him to the hospital where he is finally evaluated by our medical team.

Procedure

Some surgeons consider that the treatment of capital femoral epiphysiolysis can be carried out in some cases without surgery, i.e. by the use of mechanisms such as "skeletal traction". The drawback of this type of treatment is that it tends to be very painful for the child if not treated properly and the return time is much longer than expected.

On the other hand, those of us who support surgical treatment achieve much more favorable results while avoiding the recurrence of symptoms.

We had to operate on Manuel's hip the day after his accident in view of the number of emergencies that had occurred since the day of his arrival, considering that some emergencies take up to 7 days to be operated on due to the humanitarian collapse of our hospital we consider that its resolution was quick.

At the time of surgery and after adequate anesthesia, we locate the lesion using an image intensifier (that is what we call our portable X-ray machine), which is absolutely essential for most of the surgical treatments in our specialty.

By means of a technique of internal rotation of the leg and progressive separation we are able to reduce the injury (that is to say, to bring it to its natural position). Of course, it is necessary to stabilize it by placing a suitable material so that it can be sufficiently fastened. This was achieved by placing two screws that were placed with the help of a wire guide.

Fortunately, the surgery was successful and the skin incision was minimal, so your scar will be virtually imperceptible for the rest of your life. And although Manuel was unfortunately unable to attend this week's baseball games, his early recovery will ensure that he is properly reinstated.
