So this 68yr old woman, somebody's grandmother came to the clinic, stinking heavily of alcohol(local gin). She claimed that is what makes her strong.
Her complaint was easy satiety and abdominal discomfort. But I noticed Abdominal and leg swelling, yellowness of the eyes. "Madam have you noticed that your eyes is yellow your legs swollen?" l asked her. Quess what her response was? She said l should forget those ones that they have been there, she only came to treat the discomfort in her abdomen. Of course impression of alcoholic liver disease was made.
I told her my impression and the need to stop taking alcohol and how the whole problem could have been caused by alcohol. And she was sent for investigations.
She disappeared for two weeks only to comeback today without doing any lab testing. She was begging me to just treat her for her discomfort. How do I make her understand?
Hmmmmmm. Wellcome to Nigeria, where a patient selects which symptoms to treat and which to let be without knowing if it is a syndrome
