- Epigastric pain due to dyspepsia or acute gastritis: The man came with mid upper abdominal burning type of pain. He had some pain in the left upper part of his abdomen too.
- Acute tonsillitis with high fever: This patient was having complaints of throat pain along with high fever. On examination his tonsils were enlarged and congested.
- Severe headache with epistaxis: This was not the first time these happened to the 13 years old young boy. Every few months interval he is having similar health issues. Every time headache start first followed by bleeding from nose. I found blood pressure within normal range and no apparent nasal pathology. I gave advice the father to consult an ENT specialist for further evaluation.
- Ganglion in the left hand: A 79 years old male come with an apparent painful bony swelling in his left hand (back, against the wrist). I did X-ray to see if it was actually any bony part or not but the result came normal. It was actually a firm to hard lumps which is known as ganglia.
- A very high blood pressure: A 35 years old expatriate from Pakistan came with very high blood pressure. He already knew about his condition. I found the blood pressure as 200/130 mmHg. He was having no complaints other than headache. His lipid profile was mildly deranged and blood sugar was normal. Previously in few occasions his blood pressure was found high but this time it was the highest. He was not any antihypertensive medications. So that I started medicines and told him to consult an internal medicine specialist to search for secondary causes of hypertension.
We know that sore throat includes 3 conditions:
- Acute Pharyngitis and/or tonsillitis
- Acute tonsillitis alone and
- Quinsy (peritonsilar abscess)
Under normal circumstances, most of the sore throat are viral origin and self-limiting. Those are due to bacteria, response well to the available antibiotics. But in some cases, sore throat might not be as simple as we think. Here are few points regarding sore throat which needs to be handled urgently and with special attention.
Red flag pointers for sore throat:
- Persistent high fever
- Failed antibiotic treatment
- Medication-induced agranulocytosis (reduction of the blood cells which are fighting against various microbes)
- Mouth drooling: consider epiglottitis (don’t examine
the throat) - Sharp pain on swallowing (? foreign body)
- Marked swelling of quinsy
- Candidiasis (A fungal infection): consider diabetes or immunosuppression
