@ironshield you would want cps to fill it out, except for areas designated for family signature.
What would you do if they refuse to complete the form? @ironshield
Then have them sign the form, that they have refused. Then deny any further cooperation until the questions can be answered. Especially the judge part.
Exercise rights to: privacy, due process, not self incriminating, etc.
