Lost Requirement

I lost one of the requirement for the enrollment in Tarlac State University. So I went to the hall in Tarlac city to get the affidavit of loss and i also went to my former school, Ecumenical Christian College to get the certificate that I took the NCAE exam when I was in grade 9.
