I think by checking transactions and upvotes. Most people using two or more accounts always desire to upvote and sometimes resteem posts between the accounts.

This way you can use https://steem.supply to check the frequency of upvotes.

Transaction linking or continous upvotes by the same accounts

Voted for No way of knowing.

At least you can't know for sure unless you dig real deeper or the account owner or Someone close admitted it. You could check for things like transactions, voting patterns etc But they won't give guaranteed results.

Voted for No way of knowing.*

*Sometimes you can just presume that somebody is using two or more accounts if you can see that they are related somehow (delegation or similar) and if on different accounts you can detect the same type of behavior. For example look at @lafona account. I'm sure that if you analyze it you will find several accounts used by that same user.
