Planter fasciatis is s basically one of the most painful foot condition which affect fascia present at plantar side of foot and when this fascia become inflamed then a band of tissue forms that support arch of foot which carries body weight.
Symptoms |

• Patient who is suffering from this condition will suffer from a lot of pain and stiffness in heel and foot bottom where this fascia actually present.
• Patient will complain that his pain get worsen when he walks stands or try to run.
• Patient will tell that his pain pattern is sharp,stabbing and burning upon asking by a doctor.
• Patient will feel more discomfort and stiffness at morning time.
Causes |

• If someone encountered with repetitive strain at bottom of foot which is basically location of planter fascia then this over use can cause dis condition.
• If someone have poor mechanics of foot like any abnormality in foot or any abnormality in gait then this condition can be caused by it
• If inflammation is occurring as a result of age or any other underlying condition.
• If someone is very obese and have prolong standing hours without rest.

• If doctor is giving proper attention and proper treatment to patient then he will recover in around 6 month time period but if this condition is from long time then it can also takes 12 month for complete recovery.
• Prognosis also depends that how patient follow the guidelines of doctor like how he manage to take rest and to work in his routine.
For making diagnosis of plantar facilitis you really need to take clinical assessment patient history as well as physical examination into your consideration so here I am explaining a detailed approach and diagnostic criteria.

• You should ask patient about any previous foot injury or condition as well as surgery if your she have.
• You should evaluate symptoms of patient after asking and you should evaluate location of pain, duration and its severity.
• After evaluation of symptoms you should assess normal activity level of patient like standing hours resting hours or walking hours each day in average.
Physical Examination |

• Through visual inspection you should see that is there any redness, swelling or deformity present at ankle or foot of patient.
• You should touch at bottom of foot and at or around him and you should ask that patient is feeling pain normally or there is tenderness which feels when I touch him or her.
• You should check that is there normal range of motion at foot and at ankle joint or there is any decrease in range of motion due to swelling.
• You should assess or evaluate strength of food and ankle muscles.

• Windlass test involves to apply pressure at bottom of food or at exact location of plantar fascia while commanding the patient to flex his or her toes to check that is there any presence of pain or tenderness.
• Heel raise test can be performed by commanding patient to try to stand at their toes to check any weakness or pain presence at planter fascia.
• Talar tilt test is performed for evaluation of range of motion at ankle joint as well as for evaluation of presence of pain.

• Ultrasound and MRI both are useful for seeing people body structures that's why inflammation or any sign of damage at soft tissues can be seen by them but X-ray can only tell if there is any bony deformity or joint abnormality so most appropriate is to recommend ultrasound or MRI patient for assessing and confirmation of diagnosis.

Stretch your calf with strap according to your tolerance because excessive stretch can cause strain.
Hold it for 10-15 second and then relax

Raise heel by standing on toe for 10 seconds.
Then relax and repeat it 10 time
One by one I would like to give review of exercises that I performed;
Calf stretch |
---|
• In start three repetitions I was feeling more stretch at my calf muscles which was indicating that there is tightness present in my muscles but when I stretched then they get relaxed and at last after 10 repetitions I felt a lot of reduction in tension in calf muscles.
• I can grade its affectiveness 9 out of 10.
Heel raise |

• I felt lack of balance exercise and I was not able to hold more than 6 seconds this exercise easily but after 5 repetitions I was able to hold it for 10 seconds. I felt significant strength in my foot and ankle muscles and more ability to resist.
I can grade it's effectiveness 8.5 out of 10.
Toe curls |

• This exercise was most easy to perform for me because I have to curl my toes in this exercise just like we squeeze any bed sheet or anything through our toes. Honestly speaking I don't felt much result of this exercise in me being a healthy person but person who suffers from this condition feel significant reduction in pain and more strength in ball of foot.
I can grade it's effectiveness 8 out of 10.

You try the toe curls, calf stretch and heel raise. You did the toe curls and ball rolling correctly. But in calf stretch try to keep the band long for better stretch. In heel raise try to put all your weight on all the toes less use the metatarsophalangeal joints for better results. Always remember to cold therapy & rest is the best therapy in the treatment of plantar fasciitis. I appreciate your efforts.
