You set the amount you think (or know) you can sell, lets say 10 (the higher the number the cheaper the cost to make) but they won't start shipping until you hit your number. Once you sell 10 (or whatever number you choose, they start to ship out . Design it, market it (sell it) and they take care of the rest -- pretty sweet!
