Sleep apnea "exhausted breath pump"

The breath pump consists of breathing center, breathing muscles and lungs. If this interaction is disturbed by anatomical deformations, overweight, pulmonary diseases, neurological or muscular diseases, the breath pump no longer manages to transport sufficient oxygen into the body and the consumed oxygen ( Carbon dioxide) from the body. The respiratory muscles are exhausted by total overload. The blood gas analysis shows little oxygen and a lot of carbon dioxide.

Packed in simple words, in many cases a sleep apnea is about treating the respiratory muscles, a rest break at night to replenish the energy storage of the respiratory muscles for the day. What respiratory disease? Can be found relatively easily in the sleep laboratory today! It is usually sufficient to stabilize the respiratory muscles with a continuous positive pressure (CPAP).

When I recognized alarm signals, I took the opportunity to make a record (polygraphy) of my sleep. Here you connect various sensors that record your sleep at home. Medical practitioners can detect sleep apnea and severity in such mobile Sleep records.

I know, to a large extent, all the respiratory forms of life-sustaining machines and can also read such curves. I realized that there was a need for action in my polygraphic analysis.

A Polysomnografie in a sleep laboratory is required for a detailed sleep analysis!

After consultation with a physician, my concern was confirmed and we decided to take an immediate action with a APAP device until a place was released in the Sleep Laboratory.

My result of the polygraphy was confirmed by the polysomnography in the Sleep laboratory. The APAP device has largely eliminated obstructive apneas. However, the number of central apneas in REM sleep increased. We suspected that the prism 20a algorithm would trigger these central apneas, as we noticed that the pressure is adjusting upward, but does not lower dynamically. We switched to a APAP device "dreamstation " on the second night. This respiratory therapy unit has two pressure relief levels (C-Flex and A-flex). These settings can be used to create a "mini-BiPAP".

The morning evaluation showed that the central apneas continued to increase. In addition, almost continuous snoring was recorded. My pressure requirement was almost consistent with the upper setting of the APAP pressure. During the nighttime measurement, this caused problems with the breathing mask, so that it had to be changed. Obviously, I fought against this respiration because increased leg movements and position changes were registered.

We recognized that a CPAP therapy was not sufficient due to the high pressure requirement and decided to initiate a Bilevel therapy. Analyzing the measurement records of the last two nights, the machine weak points could be filtered to find an algorithm that dynamically adapts to my breathing pattern.

I have already had positive experiences with the APAP device Prisma 20a, so that my choice fell to the Prism 25 S/T. It contains pre-set algorithms which only have to be adapted to my respiratory disease. Base values could be derived from the last records.

Since a place in the sleep lab became vacant in the short term, I could record another night of my sleep. The breathing mask was still a problem documented by high leaks. I changed the mask again, this time to a Simplus mouth-nose mask. The result was very satisfactory, but not yet ideal. The central apneas were still present in REM sleep. During the analysis, we found that alarm reactions disturb my sleep and determined the pressure difference with 8 hpa, as set too low.

Another night, with a corrected pressure difference of 6 hpa, resulted in success. There are still central Apneas listed, but only with short phases. We have therefore found the appropriate algorithm, which also confirmed my vital values. With the Simplus-mouth-nose-mask I got the leakage in the side position as well as in the back position in the handle. A low "pressure pain" is present due to my high IPAP pressure up to 18.5 hpa. You have to put the mask on a bit more tightly so that it does not slip and blow off during mouth movements or position changes. Too high leakages can be compensated largely by the device, but also increases the ambient noise for the partner.

Since water accumulation was found in the legs and I am athletically active, I will visit a cardiologist to record a heart echo. I was already able to eliminate lung problems with a pulmonary function test. The water accumulation could also be caused only by the apneas. Therefore, rather test before I fall from the bike!

Not everyone is able to make such a detailed sleep analysis. If you feel you are suffering from sleeping sickness, talk to your doctor. Today, the sleep laboratories are highly specialized in technical and personnel. Modern devices are able to track the respiratory muscles so that an optimal gas exchange can take place. They recognize the breathing pattern and support our breathing. The biggest problem is the breathing mask. Anyone who, like me, sleeps anatomically with an open mouth, has only a small number of possibilities to find an optimal breathing mask and has to compromise. Since the proportion is about 90% nasal mask and 10% mouth-nose mask, the development focus is on new masks, probably more for nasal masks.

Untreated sleep apnea reduces your life expectancy by about 10 years.

Hi @cadgar, I will be happy to post more information. I'm just about to make myself a research object. My doctor's in. However, the clinic has no resources, so everything needs to be funded. The costs are relatively high. Let's see if I can find sponsors. I'm going to call my boss. Perhaps there is interest from this side.

It would be interesting to evaluate the different Bilevel devices (algorithms) polysomnografisch. But also full-face breathing masks. I see a lot of need for action here.

I have this condition. I find the mask and etc... intolerable. I wish I knew an other way to treat the condition. Every time I fall asleep I am taking my life in my hands.

The manufacturers have to work on the breathing masks. I just want to tell you that this is the most important part of the whole system.
There are alternatives to CPAP therapy. However, many will only help with a low sleep apnea.
