Hola a toda la comunidad, muchos pacientes llegan a la consulta con dolor y se presentan las urgencia, en la mayoria de los casos son caries muy avanzadas, que infecta a la pulpa provocando ese horrible sensación que no nos deja dormir.

Hello to the whole community, many patients come to the office in pain and the emergencies occur, in most cases are very advanced caries, which infects the pulp causing that horrible sensation that does not let us sleep.

¿Qué hacemos ante este cuadro?, se aplica el protocolo de emergencia, se hace examen clínico y se le intwrroga al paciente que nos de detalle la aparición del dolor, que tipo de dolor, intensidad y duración.

What do we do when faced with this situation, we apply the emergency protocol, perform a clinical examination and ask the patient to give us details about the appearance of the pain, its intensity and duration.

Se procede a la aplicación de anestesia, y comenzamos a eleminar el tejido carioso, en algunos casos la anestesia no produce el efecto deseado, por la cantidad de bacterias presentes, se limpia bien la cavidad y se procede a extirpar la pulpa para asi eleiminar el dolor.

We proceed to the application of anesthesia, and begin to remove the carious tissue, in some cases the anesthesia does not produce the desired effect, for the amount of bacteria present, the cavity is cleaned well and proceed to remove the pulp to eliminate the pain.

Se coloca un medicamento dentro del conducto y se remite a la realizacion de una terapia de conductos, si el paciente no se la realiza la otra opción sería la extracción, por que la infección regresa.

En otros casos el dolor es difuso y el paciente no logra identificar que diente, que esta afectado en esos casos realizamos pruebas de vitalidad, golpeamos con el mango de un instrumento a todos los dientes de lado afectado y el que reaccione ese es el que esta causando la molestia, puede reaccionar a la percusión vertical como la horizontal, otra causa son las resinas muy profundas que irrita la pulpa, por filtraciones o caries proximales y se realiza el mismo procedimiento, de extirpación de la pulpa.

In other cases the pain is diffuse and the patient is unable to identify which tooth is affected. In these cases we perform vitality tests, we hit all the teeth on the affected side with the handle of an instrument and the one that reacts is the one that is causing the discomfort, it can react to vertical and horizontal percussion, another cause is very deep resins that irritate the pulp, by leaks or proximal caries and the same procedure is performed, removal of the pulp.

Al extirpar el nervio afectado cesa de dolor y el paciente puede decidir se continua con el tratamiento endodóntico o se extrae el diente.

When the affected nerve is removed, the pain stops and the patient can decide to continue with the endodontic treatment or to extract the tooth.
