From a medical perspective there are some other things that cause really bad breath too. Most notable are diabetic ketoacidosis which make your breath smell like you're drunk (alcohol) and hepatic fetor which occurs in liver failure and makes your breath smeel like... well, poop!
You're right, these are the most common causes that threaten society. Greetings.
