A walking lady with hemoglobin 2.6 gram/dl !!

Today I got a miraculous patient in my chamber.  She was originally from Uganda, working as a house-maid in Oman for last one and half year.  

For the last 3 months, she was suffering from both legs and feet swelling, on and off. This was associated with generalized weakness, nothing else. 

On examination, I found that she was pallor and suspected that haemoglobin (Hb) might be less. But at the time of the investigation, we were shocked seeing unbelievably low haemoglobin. To be confirmed, we took a blood sample for the second time and found the exact same result. 

Usually, for ladies, the Hb count should be more than 12 gram/dl. In my clinical practice, this is the lowest Hb(2.6) I have ever seen where the patient is still walking and leading a life without any significant problem (other than leg swelling). 

The reasonable explanation from leg swelling in this patient is anaemic heart failure but there were no signs of heart failure at all. 

By the way, I referred the patient for urgent blood transfusion.
