What is Buerger's disease
Buerger's disease (thromboangiitis obliterans) is a rare arterial and venous disease in the arms and legs. In this disease, our blood vessels become inflamed, swollen and can be clogged with a blood clot (thrombi).Eventually destroys or destroys skin tissue and can cause infections and gangrene. Buerger's disease is usually first demonstrated in your hands and feet and may ultimately affect your larger arm and leg area.Almost all people diagnosed with Buerger's disease are smoking or using other forms of tobacco, such as chewing tobacco. Signs and symptoms

  1. There is recurrent acute and chronic inflammation and arterial and venous thrombosis of the hands and feet.
  2. The main symptoms are pain in the sick area, at rest and during walking (claudication).
  3. Circulatory disorders increase the sensitivity to cold.
  4. Peripheral pulses are reduced or nonexistent. There is a change of color on the extremities.
  5. The color can range from cyanotic blue to reddish blue.
  6. Skin becomes thin and shiny.
  7. Hair growth is reduced.
  8. Ulceration and gangrene in the extremities is a common complication,often resulting in slowing of limb amputation involved

Buerger's diagnosed
Buerger disease can be similar to other diseases that cause reduced blood flow to the extremities. Other abnormalities should be ruled out by evaluation, since their treatments differ substantially from Buerger's disease (for Buerger's, a treatment known to be effective is quitting smoking completely).

Like marijuana, it is also associated with vasculitis similar to Buerger or polyarteritis nodosa which should be considered in the differential diagnosis.

Angiogram of the upper and lower extremities can help in making a diagnosis of Buerger's disease. In a precise clinical setting, some diagnostic angiography findings on Buerger's. These findings include "bottle cap" arterial appearance that starts from vascular damage, especially the arteries in the wrist and ankle. Angiogram can also indicate blockage or constriction in many areas of both arms and legs.

The image below on the left is a normal angiogram. On the right, is the abnormal angiogram of the arm that shows the classic arterial "opening" appearance to the hand. This change is particularly noticeable in the blood vessels at the lower right of the image (ulnar artery distribution).

To exclude other forms of vasculitis (by excluding unusual blood vessel involvement for Buerger's), sometimes it is also necessary to perform angiograms in other body areas (eg,mesenteric angiogram).

Buerger's Treatment
Treatments that are known to be effective in Buerger's disease are a total need to stop smoking, because patients who continue to smoke generally are people who need amputation of the fingers and toes.

Despite the apparent inflammation in this disorder, anti-inflammatory agents such as steroids have not proven to be beneficial. Similarly, the strategy of blood thinning with aspirin or other agents to prevent blood clots has not been proven effective. The only way to prevent the progression of this disease is to keep away from all tobacco products.
