5 Whys - Root Cause Analysis

The 5W is a common tool used today by many businesses in finding the root cause of a problem.

A famous and historical Cathedral was rapidly deteriorating. Engineers were brought in to find the cause, they concluded that the harsh chemicals used to clean the building due to bird droppings was the cause.

What is the solution to the problem?

Why #1 – Why is the cathedral deteriorating?
Answer: Because harsh chemicals are frequently used to clean the building.
Solution? Use a less harsh chemical (does not do the job properly).

Why #2 – Why are harsh chemicals needed?
Answer: To clean off the large number of bird droppings on the building.
Solution? Build a bird proof net around the cathedral to stop them nesting, huge cost.

Why #3 – Why are there a large number of bird droppings on the cathedral?
Answer: Because the large population of spiders on and around the building is a food source for the local birds.
Solution? Regularly spray more chemicals to get rid of the spiders. Size of the cathedral will make this a costly task.

Why #4 – Why is there a large population of spiders on and around the cathedral?
Answer: Because vast swarms of insects, on which the spiders feed, are drawn to the building at dusk.
Solution? Spray the food source of the spiders, find where the insects are coming from, spray the habitat.

Why #5 – Why are swarms of insects drawn to the cathedral at dusk?
Answer: Because the lighting of the building in the evening attracts the local insects.
Solution: Change how the cathedral is illuminated in the evening to prevent attraction of swarming insects.

So, after spending a lot of money, and many months of trial and error, they finally got down to the root cause, the type of lighting used was causing the problems. A change of light globes was the inexpensive fix.

By using the simple technique of asking Why 5 times, will usually get to the root cause of a problem, with an often simple solution.

Often, we stop short of getting to the root cause, through a lack of communication, not asking enough or the right questions.

Where would that cathedral be now if we just went with all of the actions except changing the lighting source? We may spend a huge amount of time and resources on fixing the symptoms, but the problem will continually need to be addressed until the root cause is eliminated.

This is excellent. I know when I used to work in the semiconductor industry as an engineer it was not advised to bring up a problem to superiors without already having a solution. If you said something needed to be done because it would be a good idea or something was lacking, you'd be sure to be the one to be assigned and to have solved the problem.

It's not always easy, but it is best to ask the necessary questions and do the job right the first time. A slogan we used to use a lot was "fail to plan, plan to fail."

I am guessing that some organizations run on tight budget so they HAVE to be creative before implementing a "FIX" which often just moves the problem elsewhere.
I wish more people and larger organizations (government?) would be more creative and thorough when trying to fix their problems.

Why?
Well any excuse can go in here:
Something which I have heard just today:
"Because a 5 yrs old car is good only for garbage..."
"Because I don't know how to fix an old car;
"Because I would need to go to the garage to have it fix every month..."
Be smart, dig deeper and you may find a smart fix.

When used to be an auditor we would use the 5 whys all the time. Great post

But people get bugged down when we ask to many 'whys'.
How do we appro this please?

It takes a bit of practice, and a team effort to stay on track in the process of asking the 5Whys to get to the root cause.
