Difficulties in passing urine after getting an injury to the private part !!
A 10 years old Bangladeshi boy came with his father. Two days before, on last Thursday, while playing with his friends, he got a hit on his private part by one of his playmates. Later, he started having pain in his penis but passed urine normally. Urine passage was OK until today morning since when he was facing difficulties in passing the urine.
On enquiry, he denied having any hematuria (passage of blood in urine), lower abdominal pain and fever. He complained of passing urine only drop by drop with mild pain in his Penis. On questioning, he informed that he was having bowel constipation in the previous few days.
I examined his private part and found no external injuries. Then I suggested for a urine test as urinary infection may cause such difficulties, although he wasn't having any dysuria. We applied a warm water bag on his lower abdomen for a few minutes which helped him passing a good amount of urine, enough for the test. Anyway, test results came as a normal one.
Apparently, it was thought (by his family) that the injury might have caused it. They were not wrong. An injury followed by bleeding in the urethral passage or urinary bladder may cause obstruction by a blood clot. As there was no history of hematuria and urination was OK till today morning, I explained to them that injury may not be the cause here. My provisional diagnosis was "Difficulty in passing urine due to constipation".
Constipation is not a common cause of urinary difficulties or retention. But it is one of the listed causes, especially in children. I found a similar case in the Maldives.

Seems like the decision of imposing the night-time movement ban is working. No new or death cases today (Though it is hard to believe. I notified two of my patients as the suspected cases today).
