Sometimes as in the case of the little girl and her big puffy dress, some parts are too difficult to make with icing alone and a plastic figure is used. That said, I have seen some incredible sculptors make everything 100% edible. That is the ultimate goal of these artistic cakes, to make everything you see edible.

The flowers and roses start off as little lumps of icing clay. You only need a little bit to make the petals. The challenge is to make them delicate and the right colour.
